I am twisting a fringe for the first time. I like three warps twisted together better than two. Problem is I wove on 80 ends so in the middle there are only two left.- I worked in from each side.

Should I sew in an extra strand (not sure how neatly I could do this) or just twist the two that are left (leaving a thinner twist in the middle)

It sounds like you already have your answer.  After putting in so much work in a project, that little 2-strand fringe might bother you every time you look at the finished piece!  I'd take a tapestry needle and weave in under a few strand before letting it become part of the fringe.

if you go ahead and twist what's left, you'll see how it looks.  Could be fine. I've twisted fringes with one missing at the end with no problem.

I ended up putting in the extra piece of yarn. I surprised myself at how easily it went in, woven through a few picks and pulled till the end vanished.

I would never have been able to live with the odd thin bit in the middle.
